MERS/MO Goodwill Industries Case Manager-Juvenile Transition Program in St Louis, Missouri

MERS Goodwill is looking for a Case Manager to join our team located in Downtown, St. Louis. The position is full-time weekdays, has excellent benefits and will assist individuals in the Juvenile Transition Program to pursue and achieve educational and employment goals. Maintain working knowledge of community resources, document all activities in the state system, and problem solve with participants to resolve barriers to employment.

Primary Duties Include:

  • Conduct intake interviews with potential consumers, incorporating information given by other involved parties. Establish eligibility, if appropriate.

  • Establish a professional and ethical relationship, develop/maintain an individual plan for services.

  • Conduct individual/group sessions as needed for program orientation and/or Job Readiness Training.

  • Meet performance outcomes in enrollments, graduation rate, entered employment, average wage, and retention details.

  • Develop and maintain good communication and working relationships with referring staff and internal team.

  • Develop and maintain knowledge of the required outcome reporting system as required by project or grant.

  • Maintain client files with all required agency and funding source paperwork and complete all paperwork in a timely manner. Maintain confidentiality according to agency guidelines.

Qualifications:

  • HS/GED required. BS/BA and 2+ years experience in rehabilitation counseling, social work, or related field strongly preferred.

  • Valid drivers license, current auto insurance, and independent transportation required.
