*Role Purpose: *

The Diabetic Educatoris responsible for the development of patient education for the inpatient and outpatient diabetics. Specializes in working with high risk maternal patients to manage and educate diabetes and effects in pregnancy. Provides education to staff and outreach facilities on diabetes management in the maternal population. Markets and supports the diabetic clinic at the Health Center demonstrated by growth in the program.

This is in a high risk Maternal Fetal Medicine (MFM) office serving a large number of diabetic pregnant women. The Diabetic educator will provide education to patients regarding insulin/medication schedule and dosing, nutrition counseling, manage blood glucose logs and facilitate disease management in collaboration with other MFM providers. This position will fill in at MFM offices in Shiloh and Anderson, IL. as well as a couple of locations in the St. Louis area.

Responsibilities:

  • Identifies and develops new processes and therapies for monitoring and managing maternal diabetic patients. Create, develop and leads a team that will do case reviews to monitor efficacy of maternal diabetic patient therapies. Monitors and interprets the data for outcomes and provide analysis for QI.

  • Educates nursing staff about the physiology and treatment of diabetes. Visits all new inpatient diabetics. Responds to all diabetic consults. Independently reviews and analyzes patient blood sugar logs and make recommendations to the positions on insulin and medication adjustments.

  • Collaborates with the medical staff, case management, nutrition and nursing in the treatment of diabetic patients to provide the best care. Follow up with insulin pump patients and order appropriate adjustments as needed during pregnancy and in the postpartum period. Documents on the interdisciplinary care plan

  • Work with pharmacy and staff to develop appropriate diet plans and educate the OB residents, faculty and staff on nutrition guidance for high risk patients

  • Assists in the education of the diabetic family. Refers diabetic patients to other resources as needed. Coordinates the diabetic support group, providing educational opportunities as well as emotional support to the diabetic patient and his/her family in the community.

  • Attends staff meetings in the nursing care areas to educate staff. Attends all CSN meetings. Is an active participant in meetings. Provides data relevant to the diabetic education program.

Qualifications

Minimum Qualifications:

  • Bachelor's degree in a health profession

  • 3 to 5 years professional/clinical experience

  • Certified Diabetic Educator (CDE)

  • CPR Certified

  • Requires effective oral and written communication skills to communicate with customers.

  • Requires interpersonal skills to effectively deal with all levels of hospital staff, patients, families and visitors and provide leadership in sometimes difficult situations.

Department Specific : **

  • A Registered Nurse (RN) is strongly desired.

  • Experience with pregnant population is highly preferred.
